Memorial Plaque Form
Among the customs that have arisen to help us deal with loss and bereavement is the commemoration of our dearly departed on a Memorial Plaque. These plaques, which are affixed to a special memorial board in our Chapel, are an expression of our love and appreciation. On the yahrzeit of your loved one, a bulb is lit to memorialize them. Please complete and return this form to the office (for each plaque use a separate form). Once the form is received, Rabbi will review it and it will be emailed back to you for confirmation.
Members: $750.00 Non-members: $900.00

Purchase a leaf or boulder in honor of a birthday, anniversary, wedding, B'nai Mitzvah, or any other special occasion. Use the online form below to place an order. Our engraver prepares leaves/boulders 4 times per year; your order may not be ready for several weeks).
